Role Overview

The Hospice Certified Nursing Assistant is responsible for delivering personal care services under the supervision of a registered nurse. You will play a vital role in supporting our patients and their families during difficult times.

What We Offer

• Hourly Rate: $18.00 - $22.00
• Job Type: Part-Time

Key Responsibilities

• Document care provided according to organizational policies.
• Report any changes or deviations from the Plan of Care to the Case Manager.
• Safely utilize Durable Medical Equipment.
• Perform vital signs checks when indicated in the Hospice Aide Plan of Care.
• Enhance teamwork by understanding the roles of other staff involved in patient care and actively participating in team conferences.
• Monitor and provide care to prevent skin breakdown, reporting any concerns to the Case Manager.
• Assist with ambulation, transfers, and range of motion exercises as per the Hospice Aide Plan of Care.
• Offer respite to family members or caregivers when present on-site.
• Engage in Quality Assurance and Performance Improvement (QAPI) activities.
• Participate in Interdisciplinary Group (IDG) meetings as needed.

Qualifications

• Compliance with training requirements set by the state of Arizona.
• Minimum of one year of experience as a CNA, hospice aide, or home health aide.
• Must be at least 18 years old.
• Possess a valid driver’s license and an insured, reliable vehicle.
• Strong verbal and written communication skills.
• Able to work collaboratively in a team and independently when required.
